Accessing your health record
Your health record includes information like any conditions or allergies you have and any medicine you’re taking.
Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.
You can access your health record and nominate someone you trust to access it too.
You can use your NHS account (through the NHS website or NHS App) or Patient Access to access your health record.
You’ll need to ask the surgery for online access to your full health record, or you’ll only see your medicines and allergies.
Request access to your full health record online
First, you need to register for online services and prove who you are. To request access:
- visit the surgery with photo ID so we can confirm your identity
- if you are setting up access for the first time, we will give you a username and password
- go to Patient Access or the app for access
Access to other medical records
For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
If you’re a young person, you may have questions about who has access to your profile as you get older. Before a child is aged 11, their parents usually control access to their health record and online GP services. When the child is aged between 11 and 16, parents may be allowed proxy access to their child’s online services, if the GP surgery agrees this is appropriate. This access ends when the child reaches the age of 16.
If you are aged 13 or over, and you are registered at a GP surgery in England or the Isle of Man, you can access your own profile in your NHS App.
If you’re aged between 11 and 16, and you have questions about who can access your health record or online services, please contact us.
To request proxy access:
- collect a proxy access form from reception during opening times
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Private medical reports
If you are requesting a private medical report please contact us and give as much detail about what you need as possible. We aim to process this as quickly as possible but this can take 2 to 4 weeks.
Private work is done on top of NHS duties which are the GPs priority, therefore we would ask patients not to chase completion of their work within this timeframe.
